Prenatal Care Coordination



The Prenatal Care Coordination (PCC) program is a Badgercare program designed to support women throughout their pregnancy and into their baby's first two months of life. This is an unduplicated, wrap around service that augments the care provided by your physician. The goal of Prenatal Care Coordination is early entrance and continued participation in prenatal and postpartum care, to reduce low birthweight, preterm labor, and infant mortality.



Eligible Women Receive the Following Benefits:

  • Express Enrollment
  • ongoing assessment by our registered nurses and dietitians
  • client-centered health and nutrition education,
  • ongoing care coordination and monitoring
  • individualized support and services
  • assistance in accessing and obtaining needed health and social services
  • resources for available community services
  • client-based services


  • proof of pregnancy
  • is currently eligible for Wisconsin Medicaid or BadgerCare Plus
